Registering as a new patient

For any new patients wishing to register with the practice, please ensure that you live within the practice boundary outlined below - we cannot accept any patients out of area in line with our AMPS contract.

You will need to complete a registration form available from reception along with a new patient questionnaire that is provided to you. Please complete and return the form along with photo ID and proof of address for anyone over 16; anyone over 16 must attend themselves with the ID, this cannot be brought in on their behalf. If you are unable to provide ID/proof of address, please speak with the Office Manager.

Please be aware that patients are no longer registered to a specific doctor within the practice. Patients are registered with the practice as a whole and therefore can see any member of staff within the team.

The Practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.

If you have provided a mobile number on your registration form, please ensure that you indicate on the new patient questionnaire if you give consent to be contacted via text message - this is the same if you provide an email address.  

Registration Complete

Once you have been registered at the surgery, you should receive a text letting you know that you are now a patient of Willaston Surgery. When availability allows, the surgery team will be in touch to arrange a New Patient Health Check. 

We welcome patients from within the practice area, which covers the postcodes featured below.

We do not accept patients to our list from outside these areas and if you move out of our area you will be asked to register with a practice closer to your address within 2 weeks, after which time you will be removed from our patient list. NHS England have given practices the freedom to register out of area patients and patients who work in the local area but do not reside there. However, this is at every practices discretion and unfortunately it is Willaston Surgery’s policy to stay within the listed boundaries, therefore we will NOT be registering anyone out of area. If you have any enquiries please contact NHS England to discuss further. 

Anyone currently registered with the practice will continue to receive a full service in the mean time
